New Patient Registration

Complete the application form available at reception. It is helpful, although not essential , to bring your NHS medical card if you have one

You will be asked to make an appointment for a new patient health check

It is not necessary to always attend the GP with whom you are registered – you may choose to see any of the doctors, and whilst we make every effort to comply with patients’ requests for a preferred doctor this may not always be possible

Temporary residents

If you are away from home and need medical attention you are entitled to see a doctor anywhere in the UK - you will be asked to complete a form which indicates your home address and GP

Non-English Speakers

Pre-registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all the details, click on the "Send" button to mail your form to us. Once we have this form we will be able to register you but only as an 'Incomplete Registration'.  In order to fully activate your registration you will need to visit the surgery to give proof of identity and address.  Only then will your registration be activated and sent to the Health Authority.

Medical Questionnaire

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
